We are still keeping some paper, but the only form you actually need to submit is the Volunteer Disclosure Statement

• Display and print out the whole form 
• Each form MUST be only ONE page long, otherwise not acceptable
• Make sure to fill out the form completely
• Submit the form on registration day;  after registration day, forms should be submitted to the appropriate division coordinator or contact a club officer

	Volunteer Form				This form needs to be filled out for all volunteer positions (i.e. coaches, coordinators, etc.).  In addition, all volunteers must also fill out the Volunteer Disclosure Statement.  Check online registration

	Sponsor Contract				Anyone wanting to sponsor a team should fill out this form and submit it to the Sponsorship Coordinator as noted on the form with the respective sponsorship fee.  Check online registration

	Volunteer Disclosure Statement				This form needs to be filled out for all volunteer positions. Due to the alarming number of incidents involving adults and youths under direct adult supervision, VRYSA has implemented US Youth Soccer's Kidsafe Program whereby all volunteers are required to complete this form.  The intent of the procedure is to ensure the protection of our volunteers and our youth.  Please be assured that maximum confidentially will be maintained.
